Eligibility for Services

To receive services at Vitality IV Drip, you must:

• Be at least 18 years of age (or have parental/guardian consent)
• Provide accurate and complete health information
• Disclose all current medications, allergies, and medical conditions
• Not be under the influence of alcohol or drugs during treatment
• Follow pre-treatment and post-treatment instructions

Informed Consent

Before receiving any IV therapy treatment, you will be required to sign an informed consent form. This form will explain the nature of the treatment, potential risks and benefits, and alternative options. You have the right to ask questions and receive answers before consenting to treatment.

Appointment Policy

Scheduling

Appointments can be scheduled online, by phone, or in person. We recommend booking in advance to ensure availability at your preferred time.

Cancellation Policy

We require at least 24 hours notice for appointment cancellations or rescheduling. Late cancellations or no-shows may be subject to a cancellation fee of up to 50% of the scheduled service cost.

Late Arrivals

If you arrive more than 15 minutes late for your appointment, we may need to reschedule your treatment to accommodate other patients.

Payment Terms

Payment is due at the time of service unless other arrangements have been made. We accept cash, credit cards, and debit cards. Prices are subject to change without notice.

IV therapy services are generally not covered by health insurance. We do not bill insurance companies directly. Upon request, we can provide documentation for you to submit to your insurance provider for potential reimbursement.

Membership Programs

Membership programs are subject to separate terms and conditions. Memberships are non-transferable and non-refundable. Monthly membership fees are charged automatically to your payment method on file. You may cancel your membership at any time with 30 days written notice.

Refund Policy

Due to the nature of medical services, we generally do not offer refunds for completed treatments. If you are dissatisfied with your service, please contact us within 48 hours to discuss your concerns. Refunds for unused services or packages may be considered on a case-by-case basis.

Assumption of Risk

While IV therapy is generally safe when administered by qualified professionals, all medical procedures carry some risk. Potential risks of IV therapy include but are not limited to:

• Bruising, swelling, or discomfort at the injection site
• Infection at the injection site
• Allergic reactions to ingredients
• Vein inflammation (phlebitis)
• Air embolism (rare)
• Fluid overload (rare)

By receiving treatment, you acknowledge these risks and agree to hold Vitality IV Drip harmless for any adverse effects that may occur despite proper administration of care.

Limitation of Liability

To the maximum extent permitted by law, Vitality IV Drip and its owners, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or website.

Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific service giving rise to the claim.
